29 CFR 1910.132(a): Protective equipment was not used when necessary, whenever hazards capable of causing injury and impairment were encountered: (a) Warehouse, Conveyor Line 2: On or about July 2, 2012, Employees were not provided protective equipment to prevent them from falling while performing maintenance work from a temporary elevated working platform. An employee fell approximately 23', resulting in disabling injuries. OR IN THE ALTERNATIVE 29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: (a) Warehouse, Conveyor Line 2: On or about July 2, 2012, Employees working from a temporary elevated working platform were not provided standard railings and toeboards to protect them from falling while performing maintenance work. An employee fell approximately 23', resulting in disabling injuries.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a)Warehouse, Denester Machine: Operators and other employees working in this area were not protected by the use of guardrails or gates from the exposed moving rollers. Operators and other employees can trip and fall into the moving rollers, exposing them to severe injuries, observed on or about 7/17/12.
