This violation was observed on or about July 18, 2012, in the machine shop where employees operating a HE&M Bandsaw were exposed to a struck-by hazard from the rotating blade while cutting metal parts without a guard.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. The employer does not ensure that each operator has successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. This violation occurred on or about July 18, 2012, in the machine shop area where employees, who operated a Yale forklift to transport metal products, were not trained and evaluated on the safe operation of the forklift.

This violation occurred on or about July 18, 2012, in the machine shop where employees were using oils and lubricants in the manufacture of metal products and safety data sheets for the chemicals were not available, nor was the employees trained on hazardous communication.
