(b) Throughout the facility: On or about 18 July 2012 and at times prior, access to the transformer, equipment breakers and lighting panels were blocked by storage. Examples include but are not limited to: (a) in the framing building the lighting and breaker panels were blocked by storage boxes containing frame corner molding samples, (b) in the finish shop access inside the partitioned room containing the transformer and the panels containing equipment disconnects and lighting breakers was blocked by storage.

29 CFR 1910.305(g)(1)(iv)(B):     Flexible cords and cables were run through holes in walls, ceilings, or floors:    (a) Finish Shop: On or before 18 July 2012 and at times prior, flexible cords were run thorough walls at locations including but not limited to the south side of the Speedaire compressor room and also inside that same compressor room a flexible cord was knotted around the electrical conduit and then it passed through the north wall of the room. Employees were exposed to shock hazards from temporary wiring hidden behind a wall and it was not known if the cord had sustained damage.
