29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips: a) On or about July 24, 2012, and times prior thereto, the rotating chuck on the Delta drill press was not guarded. The drill press serial number #9151. Employee (s) exposed to a caught in hazard.

29 CFR 1910.119 (d)(3)(ii): The employer did not document that the equipment in the process complies with recognize and generally accepted good engineering practices: a) On or about July 24, 2012, and times prior thereto, the piping in the plant had no directional flow indicators marked on the piping. In the event of an emergency and/or maintenance there was no visual way to know what way the flow was going.
