29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)    On or about July 12, 2012, at 2404 Meadow Church Road, Duluth, Georgia:    a) An employee involved in residential construction activities was exposed to a fall hazard of approximately 12.6-feet when he was installing decking on the roof of a house without using fall protection.     b) Employees involved in residential construction activities were exposed to a fall hazard of approximately 11.2-feet while framing the second floor of a house at an unprotected edge.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    On or about July 12, 2012, at 2404 Meadow Church Road, Duluth, Georgia:    Employees engaged in residential construction work were exposed to a fall hazard of approximately 11.4-feet when they used a 20-foot aluminum extension ladder to access and egress the second floor of a house without extending the ladder 3-feet above the second floor landing.

29 CFR 1926.1060(a)(1): The employer did not provide a training program for each employee using ladders as necessary, enabling each employee to recognize hazards related to ladders and the procedures to be followed to minimize these hazards.    On or about July 12, 2012, at 2404 Meadow Church Road, Duluth, Georgia:    Employees engaged in residential construction activities were exposed to a fall hazard of approximately 11.4-feet when they used a 20-foot aluminum extension ladder to access and egress the second floor of a house. The employer did not train the employees how to recognize and minimize hazards associated with ladder use on the job.
