29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards as employees were operating a powered industrial truck in a storage yard:     (a)  Storage Yard, on or about July 20, 2012, and at times prior thereto, the older model Clark Yarlift, 15,000 pound capacity, was not provided with a seat belt for employees to wear to prevent them from being thrown from the vehicle.      Note:  Abatement certification is required for this item.

29 CFR 1910.252(b)(4)(v):  When arc welding is to be suspended for any substantial period of time, all electrodes were not removed from the holders and the holders carefully located so that accidental contact cannot occur and the machine disconnected from the power source.    (a)  Welding Area, on or about July 20, 2012, and at times prior thereto, a 6011 welding rod was left in the holder, unattended.    Note:  Abatement certification is required for this item.
