The employees working inside the building removing demo material, insulation and cleaning the area were not provided overhead protection from the metal pipes, metal heating and air housing unit, and other materials hanging loosely from above.

The employees were using the damaged stairway to access and exit the unprotected side of the roof at a height of 15 feet from the lower level.

29 CFR 1926.851(b):  All stairs, passageways, ladders and incidental equipment thereto, which are covered by this section, shall be periodically inspected and maintained in a clean safe condition.    On or about Wednesday July 11, 2012 on the side of the building at 212 Railroad Ave Jackson, GA 30233;    a). The employees were using the damaged stairway that was not inspected, where the tree roots separated the upper platform from the building structure. The employees used the stairway to access and egress the roof of the building at about 15 feet above the ground level.
