29 CFR 1910.132(d)(1)(i):     The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:    a) Jobsite: FCC Unit, Monroe Refinery, 4101 Post Road - On or about 7/25/2012, appropriate personal protective equipment was not selected nor required to be used to protect the affected workers while performing chemical equipment setup and chemical cleaning at the FCC Unit.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
