29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph (b). Note: If using these systems was infeasible or created a greater hazard, the employer did not develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502: a) JD Mountain Drywall, LLC at 2390 - 2396 Uinta Street, Denver, CO 80238: On or before July 28, 2012, the employer did not ensure that employees were protected from fall hazards caused by three window wall openings, with the bottom ledge 25" above the second floor, and no upper guardrails were in place. Two windows were 5' wide, and a third was 2' wide. Condition exposed employees to fall hazards of 20'.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage: a) JD Mountain Drywall, LLC at 2390 - 2396 Uinta Street, Denver, CO 80238: On or before July 28, 2012, the employer allowed employees to use a red extension cord, with damaged outer insulation, to power a portable 110 volt electric drill. Condition exposed employees to electrical shock hazards.
