Employees installing windows from the elevated platform were exposed to an unprotected fall to the ground below.

29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:        On or about July 25, 2012, the employer did not ensure employees working from an elevated scaffold platform was attached to the forklift. Employees installing windows at a height of up to 15 feet were exposed to an unprotected fall to the ground below.

29 CFR 1926.454(a)(3): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the proper use of the scaffold, and the proper handling of materials on the scaffold.         On or about July 25, 2012, the employer did not ensure employees installing windows while working from an elevated scaffold were trained to recognize hazards associated with the scaffold in use. Employees were exposed to an unprotected fall of over 15 feet to the ground below.
