29 CFR 1926.601(b)(5): Motor vehicle(s) had cracked or broken glass that had not been replaced:    (a) Trench/Excavation located at 2550 Spring Road, Smyrna, GA 30080: Employees working in and around the trench area were exposed to struck-by hazards while the foreman operated a CAT 305Ccr Mini-Excavator with a broken front windsheild in the cab. The entire front glass panel had a spider-like crack obstructing the operator's view.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    (a) Excavation located at 2550 Spring Road, Smyrna, GA 30080: Employee working in an unprotected excavation was exposed to possible cave-in hazards in a 25' long x 7' wide x 6' deep trench which was not provided with a safe means of egress, i.e. ladder and or ramp.

29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:    (a) Excavation located at 2550 Spring Road, Smyrna, GA 30080: Excavation was not inspected by the competent person on site prior to the start of work. Employees were exposed to cave-in hazards when a cave-in protective system was not used and a ladder for safe egress was not provided for employees installing 6" pipe in an excavation that measured 25' long x 7' wide x 6' deep.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that the excavation was sloped at an angle steeper than one-half horizontal to one vertical.    (a) Excavation located at 2550 Spring Road, Smyrna, GA 30080: Employee was exposed to cave-in hazards while engaged in sewer line installation work in an excavation that was 25' long x 7' wide x 6' deep with no protective system, i.e. trench box, trench sheild, shoring and or sloping in place to prevent collapse.
