Jobsite:    A synthetic web sling used to lift the load had snags and broken threads along its edge.

Jobsite:    A loose piece of bridge decking was not protected from unintentional displacement when it was not secured by slings or other rigging to the rest of the load.  The unsecured decking exposed workers to a struck-by hazard.
