29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a)  Q Energy Services, LLC at 6450 Holly Street, Comerce City, CO 80022:  On or before July 27, 2012, the employer did not ensure that employees were protected from falling when installing solar panels on a flat school roof.  Although a parapet wall was in place, it varied in height from 29" to 40" high, and did not meet standard guardrail requirements of 42" +/- 3".  No other plan for fall protection was in place.  Condition exposed employees to fall hazards of up to 20'6".
