29 CFR 1926.453(b)(2)(v):  A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    On or about July 31, 2012, on the west side of the building, employees were working from the basket of a Genie aerial lift while performing waterproofing work and were not tied off to the basket.  The employees were exposed to falls of approximately 25 feet to the concrete below.
