29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    At the jobsite, on the roof, on or about 7/19/12:  Employees were performing roofing work on a 12:12 pitch roof with approximately 15 feet ground to eave height, without fall protection of any kind.    Abatement certification must be submitted for this item.

29 CFR 1926.503(a)(2):     The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)    At the jobsite, on the roof, on or about 7/19/12:  Employees were performing roofing work on a 12:12 pitch roof with approximately 15 feet ground to eave height, without fall protection of any kind and without having been trained on the hazards and protections associated with falls in construction and with the correct use of fall protection systems.    Abatement certification must be submitted for this item.
