29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:                                                                                                                         a)  West side of building, 223 Clever Road, McKees Rocks, Pennsylvania 15136:  Observed on or about August 3, 2012, employees were working from the basket of a Genie S-60 aerial lift while not wearing fall protection.  The employees were exposed to a fall hazard of approximately 20 feet to the ground below.

29 CFR 1926.1053(b)(4):  Ladder(s) were used for purposes other than the purpose for which they were designed:    a)  West side of building, 223 Clever Road, McKees Rocks, Pennsylvania 15136:  Observed on or about August 3, 2012, a stepladder was being used while not in the open position.  Employees using the ladder to access the first level of the building were exposed to a fall hazard of approximately 6 feet 2 inches to the ground below.
