29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: (a) Construction Site: On or about August 2, 2012, and at times prior thereto, employees were exposed to a fall of approximately thirty feet to the ground below while installing flashing to the top a thirteen inch parapet wall. (b) Construction Site: On or about August 2, 2012, and at times prior thereto, as the controlling employer, ABC Roofing did not ensure its sub-contractor, Great Falls Roofing & Painting Inc., protected their employees in that employees were exposed to a fall of approximately fifteen feet to the ground below while roofing the top of an elevator.

29 CFR 1926.503(a)(1): The employer did not provide an effective training program for each employee who was exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: (a) Construction Site: On or about August 2, 2012, and at times prior thereto, employees were exposed to a fall of approximately thirty feet to the ground below while installing flashing to the top a thirteen inch parapet wall without adequate training in the recognition of fall hazards and the means to control these hazards. (b) Construction Site: On or about August 2, 2012, and at times prior thereto, as the controlling employer, ABC Roofing did not ensure its sub-contractor, Great Falls Roofing & Painting Inc., protected their employees in that employees were exposed to a fall of approximately fifteen feet to the ground below while roofing the top of an elevator without adequate training in the recognition of fall hazards and the means to control these hazards.
