29 CFR 1926.100(a):     Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.    a.  On or about 8/1/2012, North Parking Lot, A protective helmet was not worn by an employee who stood in an area  below a scissors lift to hand wood sheeting to workers in the scissors lift which exposes the employee to struck-by hazards.

29 CFR 1926.451(g)(4)(iv):     Midrails were not installed at a height approximately midway between the top edge of the guardrail system and the platform surface:      a. On or about 8/1/2012, North Parapet, The midrail chain at the scissors lift gate was not connected to its attachment point and two employees who stood in the scissors lift platform to install wood sheeting were exposed to falls from elevations of greater than 10 feet.

29 CFR 1926.501(b)(2)(i):     Each employee who was constructing a leading edge 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:    a.  On or about 8/8/12, North Roof, Two employees installing wood sheeting to roof arches stood on the top surface of roof arches without any type of fall protection and were exposed to falls from elevations greater than 6 feet.
