29 CFR 1926.453(b)(2)(v):  A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:             a.  On or about 8/3/12, Shopping Center Parking Lot, a personal fall arrest or restraint system was not worn by an employee when working from the basket of an aerial lift to string telecommunications cable which exposes the employee to falls from elevations greater than 6 feet.
