29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a)  Carlos Enterprises LLC at 8145 Raspberry Drive, Frederick, CO 80504:  On or before August 4, 2012, the employer did not ensure that employees wore safety glasses when using or working in the vicinity of pneumatic nail guns.  Condition exposed employees to the hazard of projectiles to the eyes.

Note:  If using these systems was infeasible or created a greater hazard, the employer did not develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502:    a)  Carlos Enterprises LLC at 8145 Raspberry Drive, Frederick, CO 80504:  On or before August 4, 2012, the employer did not ensure that employees used fall protection when shingling this ranch style home with a 6 in 12 pitch roof.  Condition exposed employees to fall hazards of up to 10'.
