29 CFR 1910.334(a)(3)(ii): Attachment plugs and receptacles may not be connected or altered in a manner which would prevent proper continuity of the equipment grounding conductor at the point where plugs are attached to receptacles. Additionally, these devices may not be altered to allow the grounding pole of a plug to be inserted into slots intended for connection to the current-carrying conductors. a) On or about August 7, 2012; in the HR Managers office, employees were allowed to utilize and work near a metal table fan which had a plug that was missing a ground pin which prevented the fan from being effectively grounded.

29 CFR 1910.37(b)(4): If the direction of travel to the exit or exit discharge is not immediately apparent, signs must be posted along the exit access indicating the direction of travel to the nearest exit and exit discharge. Additionally, the line-of-sight to an exit sign must clearly be visible at all times. a) On or about August 7, 2012; in the receiving and assembly areas at the rear of the building, employees were allowed to work in an area where the direction of travel to the exit discharge was not immediately apparent.
