29 CFR 1926.21(b)(6)(i): The employer shall comply with any specific regulations that apply to work in dangerous or potentially dangerous areas: (a) On or about 08/08/12 - At the job site located at the intersection of Forrest Ave and Grady St. (Manhole number 29-9), Gadsden, AL, an employee working in the bottom of a active sewer line manhole had entered to cut the head of the sewer line. A gasoline powered chain saw was used. Monitoring had not been conducted prior to entry and no personal protective equipment was provided.
