29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: Employees operating equipment used to cut and compact metal were exposed to amputation hazards because the machines were not guarded. 1. The Mosley shear (Mini Monster) used to cut metal was not guarded 2. The northernmost Fox Baler (model no. 60, serial no. 847866) used to compact metal for recycling was altered so that the safety gate was disabled and wired in the up position during use. 3. The southernmost Fox Baler used to compact metal for recycling was altered so that the safety gate was disabled and wired in the up position during use.

29 CFR 1910.305(b)(2): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: Employees working in the shop area near the Mosley shear were exposed to fire hazards because an electrical outlet box on the south wall on the east side of the loading docks was missing the cover.
