29 CFR 1926.404(b)(1)(i): General. The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors. (a). Worksite located at 1 NE Vivion Road, Kansas City, Missouri- Employees were exposed to a shock or electroctution hazard in that the employees were using extension cords and tools that were not protected by the use of a ground fault circuit interrupter.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. (a). Worksite located at 1 NE Vivion Road, Kansas City, Missouri- Employees were exposed to a shock or electrocution hazard in that an extension cord was run from the interior of the building to the exterior in which the metal door frames were smashing the cord and could cause a breach in the outer insulation and a potential energization of the door frame.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). (a). Worksite located at 1 NE Vivion Road, Kansas City, Missouri- Employees were exposed to a fall hazard in that the employees were observed working out of an aerial lift without the use of any means of fall protection.
