29 CFR 1910.178(l)(2)(ii):     Training shall consist of formal instruction that is provided to each operator consisted of formal instruction (i.e. classroom, video tape, etc.) practical training (i.e. demonstrations by trainer & exercises performed by trainee) and an evaluation of the operators performance in the workplace, prior to permitting an employee to operate a power industrial truck:    The employer is failing to ensure that all employees operating powered industrial trucks at the facility are trained in accordance with this subpart in that employee(s) are not receiving training on the operation of the Caterpillar fork lift in the workplace.
