(a)  Denver Hardwood Co., 5151 Franklin St., Denver, CO 80216:  On and before August 13, 2012, the exposing employer failed to ensure that a cover for a 110 V junction box located near the entrance to the main warehouse, and mounted a sheet of plywood, was securely fastened.  Condition exposed employees to electric shock hazards.
