29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Die Casting Production Area: On or about July 27, 2012 and at times prior thereto; the employer allowed employees to be lifted and supported by the forks while in a wire shipping basket. The design and use of the basket were not in compliance with ANSI B56.1 requirements for elevating personnel and/or approved by the powered industrial truck manufacturer.

29 CFR 1910.178(l)(3)(i)(G): Powered industrial truck operators did not receive initial training on fork and attachment adaptation, operation, and use limitations: a) Powered Industrial Trucks (forklifts): On or about July 27, 2012 and at times prior thereto; the employers forklift training did not enable the forklift operator to recognize the hazards with using a wire shipping basket as a personnel lifting platform and the precautions and requirements for using a personnel lift with a forklift which exposed an employee to falls up to 13-feet to the ground below.
