29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 paragraph (b) or (c):    The employer does not protect its employees from cave-ins while working in a trench.  This violation was observed on or about June 11, 2012, at the southeast corner of I-45 and Pruitt Road, Spring TX, where employees were exposed to a struck-by hazard when working inside of a trench excavation without an adequate protective system.
