The employer failed to properly mount / install three electrical boxes (receptacles) per listing and labeling instructions and the NEC code.

29 CFR 1926.405(b)(1): Conductors entering boxes, cabinets, and fittings were not protected from abrasion:  On or about 08/08/12 at Conyers, GA: Employees were exposed to electrical hazards when accessing a power panel located on the general contractor's office trailer.  The employer failed to provide adequate strain relief to the electrical cord that entered the bottom of the power panel.

On or about 08/08/12 at Conyers, GA: Employees were exposed to electrical hazards when accessing circuit breakers located on the temporary circuit breaker panel.  The employer failed to ensure that the circuit breaker box was dead front in order to prevent employee exposure to live electrical parts.
