The employees were exposed to a 17 feet 10 inch fall hazard.

29 CFR 1926.451(b)(1)(ii): The platform was not planked or decked as fully as possible and the remaining open space between the platform and the uprights exceeded 9.5 inches (24.1 cm): a) Jobsite, Hyundai Dealership, 352 Pittsburgh Road, Butler, PA: On or before 08/13/12, the work platform on a fabricated frame scaffold used by employees to install gypsum board on the side of the building was not fully planked. There was a 28 inch opening in the work platform which exposed the employees to a 17 foot 10 inch fall hazard.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access: a) Jobsite, Hyundai Dealership, 352 Pittsburgh Road, Butler, PA: On or before 08/13/12, employees working 4 feet above a point of access on a fabricated frame scaffold were not provided a ladder or other direct access. The Werner extension ladder was not extended to provide direct access for the employees working on the top platform.
