29 CFR 1926.501(b)(10):     Ech employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.     (a) Location - 2087 Main Street, Tewksbury, Ma:   On or about 7/31/2012, workers on a roof that was greater than 6 feet above a lower level were not protected from falling to the ground below.

29 CFR 1926.1053(b)(1):     When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.    (a) Location - 2087 Main Street, Tewksbury, Ma:   On or about 7/31/2012, workers accessing a roof using a ladder that did not extend three feet beyond the roofs edge.
