29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) Huracha Construction at 1921 80th Ave., Greeley, CO 80634: On or before August 11, 2012, the employer did not ensure that employees wore safety glasses when using or working in the vicinity of pneumatic nail guns. Condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph (b). Note: If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502: a) Huracha Construction at 1921 80th Ave., Greeley, CO 80634: On or before August 11, 2012, the employer did not ensure that employees wore fall protection when shingling this two-story single family home with a 6 in 12 pitch, when the employees were working on the lower roof. Condition exposed employees to fall hazards of 11 feet.
