29 CFR 1926.651(i)(3):     Sidewalks, pavements and appurtenant structure shall not be undermined unless a support system or another method of protection is provided to protect employees from the possible collapse of such structures.    a. On or about August 17, 2012 - at the above addressed jobsite, employees conducting plumbing operations in an excavation that measured approximately seven (7) feet in depth were not protected from struck by hazards due to undercut street pavement not being supported.

29 CFR 1926.652(a)(1):     Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when:    a. On or about August 17, 2012 - at the above addressed jobsite, employees performing plumbing work in an excavation seven (7) feet in depth were not provided with an adequate protective system, thereby exposing the employees to the hazards associated with cave-ins.
