29 CFR 1926.602(c)(1)(viii)(A): The safety platform was not firmly secured to the lifting carriage and/or forks.    At 15095 W. 151st St, Olathe, KS, the Arrow Manufactured Personnel Platform lifted by a forklift was not secured to the lifting carriage and forks in that the security cable was not securely closed by the provided padlock, exposing personnel to a potential fall hazard of approximately 15 feet to the concrete below.
