29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors. Sources of ignition include but are not limited to open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat: At the worksite: On or about August 11, 2012, and at times prior thereto, the employer did not take adequate precautions to prevent the ignition of flammable vapor by placing a diesel engine powered mud pump unit next to the skid mounted mud tank. The mud pump and mud tank were being used to blend/mix crude oil and condensate. Employees were exposed to hazards associated with fire and explosion.
