29 CFR 1926.900(h): When blasting is done in congested areas or in proximity to a structure, railway, or highway, or any other installation that may be damaged, the blaster shall take special precautions in the loading, delaying, initiation, and confinement of each blast with mats or other methods so as to control the throw of fragments, and thus prevent bodily injury to employees.    a) Ancillary 2 escalator shaft:    Blasting operations to excavate Ancillary 2 escalator shaft were done in areas with congested street and vehicle traffic and in proximity to low-rise buildings. The employer did not take precautions in the loading and confinement of blast #73 with mats or other methods to sufficiently confine the blast. When blast #73 fired off, upward blast energy threw rock fragments from shaft opening into the jobsite at street surface, public street areas, and nearby structures. On or about 08/21/12.    NOTE:  IN ADDITION TO CERTIFICATION OF ABATEMENT, THE EMPLOYER IS REQUIRED TO SUBMIT DOCUMENTATION OF ABATEMENT FOR THIS ITEM.
