29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    a) Second Floor Center - Laborers and masons were observed near a floor hole that measured 32 inches by 47 inches, and exposed to a 10 foot fall hazard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) Second Floor Center - Laborers and masons were observed using a portable metal ladder to gain access to the second floor and the side rails only extended 11 inches above the landing.
