Worksite -  The scaffold was used with different manufacturers components in that one aluminum pick was supported by a second aluminum pick on the warner ladder rung and the pump jack scaffold which was of a different manufacturer thereby exposing employees to falls of 21 feet in the event of ladder, pick or scaffold failure.
