29 CFR 1926.451(c)(2)(iv):     Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:    On or about August 21, and at times prior thereto, at the job site located at 1964 E. Grand in Hot Springs, Arkansas. The employer allowed employees to be elevated with a forklift mounted platform that was not approved by the forklift manufacturer.

29 CFR 1926.451(c)(2)(v):     The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:      On or about August 21, 2012, and at times prior thereto, at the jobsite located at 1964 E Grand in Hot Springs, Arkansas. Employees were working from a forklift mounted platform that was not securely attached to the forks.
