29 CFR 1926.21(b)(6)(i): Employees required to enter into confined or enclosed spaces were not instructed as to the nature of the hazards involved, the necessary precautions to be taken, and in the use of protective and emergency equipment required:    a) On or about August 15, 2012-Job Site at Beach 25th Street and Aztec Place Far Rockawasy, NY 11691    Entrant was not wearing a harness in a permit required confined space.      b) On or about August 15, 2012-Job Site at Beach 25th Street and Aztec Place Far Rockawasy, NY 11691    Entrant modified the retrieval system placing a 3-foot sling between the harness tripod.    c)  On or about August 15, 2012-Job Site at Beach 25th Street and Aztec Place Far Rockawasy, NY 11691    Entrant did not know the proper way to put on a harness as it was inside out.    d) On or about August 15, 2012-Job Site at Beach 25th Street and Aztec Place Far Rockawasy, NY 11691    Attendent/entrant was performing other duties such as removing mud and debris and empting buckerts 40-feet awasy from the manhole.

Emplyees reserfacing roads, building trench boxes, and confined space entry without advanced warning signs identified in the M.P.T. Plan.    b) On or about August 15, 2012-Job Site at Beach 25th Street and Aztec Place Far Rockawasy, NY 11691      Job #SEQ200523 construction of storm & sanitary sewers & appurtenances in New Haven Ave Borough of Queens.    Employee flagging at multiple locations through the worksite did not have required flagger ahead sign.      WRITTEN ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
