29 CFR 1926.405(a)(2)(ii)(I):     Flexible cords and cables used for temporary wiring were not protected from damage:    a) DeSalvo Construction Company, Inc./ Panera Bread Location / 4014 Peach Street, Erie, PA / Black colored power cable: On or about August 21, 2012 construction site vehicles drove over the electric cable that provided temporary power to the interior of the building. In addition, the cord was going through a closed doorway and encountering additional pinching hazards.     b) DeSalvo Construction Company, Inc. / Panera Bread Location / 4014 Peach Street, Erie, PA / Black colored power cable: On or about August 21, 2012 construction site vehicles drove over the electric cable that provided temporary power to the interior of the building. In addition, the cord was going through a closed doorway and encountering additional pinching hazards.    c) DeSalvo Construction Company, Inc. / Panera Bread Location / 4014 Peach Street, Erie, PA / Yellow colored extension cord: On or about August 21, 2012 contractors drove over the extension cord going from the electrical panel plug outlet to the interior of the building. In addition, the cord was going through a closed doorway and encountering additional pinching hazards.

29 CFR 1904.29(b)(1):     The employer did not provide the required injury or illness descriptions on the Log of Work-Related Injuries and Illnesses, OSHA Form 300:    a) DeSalvo Construction Company, Inc. / Panera Bread / 4014 Peach Street, Erie, PA: The entries in Column F on the OSHA 300 Log for Calendar Year 2012 were not descriptive in that the agent, object, and/or substance that directly injured or made the person ill was not noted/documented. The injury dates are 4/17/12, 6/25/12, 8/2/12, 8/9/12, 8/20/12 and 8/28/12.
