29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    On or about 08-17-12, at 1200 East Las Olas Boulevard Fort Lauderdale, FL 33301, an employee was exposed to a fall hazard of approximately 14 feet to ground level, while standing on the mid rails of a scissors lift without fall protection.    REPEAT: LOTSPEICH WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.451(g)(1)(vii) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313109878, CITATION NUMBER 1, ITEM NUMBER 4, AND WAS AFFIRMED AS A FINAL ORDER ON May 12, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 11701 LAKE VICTORIA GARDENS AVE., PALM BEACH GARDENS FL. 33418.
