On or about August 23, 2012 in the Chemistry Lab utility room, ceiling tiles were damaged by water intrusion which could support mold growth.      b.  On or about August 23, 2012 in the Chemistry Lab rest room, ceiling tiles were damaged by water intrusion which could support mold growth.

29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition:        a.   On or about August 23, 2012 in the Chemistry Lab, the floor on the south side of storage cabinet #3 had liquid on it.  This condition exposed employees to the hazard of slips and falls.    b. On or about August 23, 2012 in the Biology Lab restroom, the floor had been wet due to water intrusion and floor tile were coming up.  This condition could result in mold growth and hazards such as slips, trips and falls.

On or about August 23, 2012 in the Chemistry Lab, at the exit route from lab hoods #2 and #5, stored equipment such as but not limited to an oven on a cart partially blocked the exit route.     b.  On or about August 23, 2012 in the Chemistry Lab, at the exit route just north of the utility room, stored equipment partially blocked the exit route.      These conditions exposed employees working in the chemistry lab to the hazard of delaying their exit in the event of an emergency such as fire or chemical spill.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a.   On or about August 23, 2012, access to the emergency eyewash station in the Chemistry Lab was blocked by equipment.  Employees working with acids were exposed to the hazard of acid splashing into their eyes without being able to immediately flush their eyes with water.    b.  On or about August 23,2012, access to the emergency shower station in the Chemistry Lab was blocked by equipment.  Employees working with acids were exposed to the hazard of spilling or splashing acid onto their arms, legs or torso without being able to immediately rinse their body with water.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:      a.   On or about August 23, 2012 and prior thereto, in the Chemistry Lab at the aisle just north of the utility room two metal outlet boxes with knockouts were suspended from the ceiling by a flexible cord.       b.  On or about November 15, 2012 and prior thereto, in the Chemistry Lab just south of the lab hoods a metal outlet box with knockouts suspended from the ceiling by a flexible cord was sitting on a desk.     c.  On or about November 15, 2012 and prior thereto, in the Chemistry Lab between the flammable storage cabinet and the lab hood on the north end of the west wall was a metal outlet box with knockouts lying on the floor.     These conditions exposed employees holding the boxes while plugging flexible cords in and unplugging flexible cords to  the hazard of pushing the knockouts in and contacting live electrical parts.

29 CFR 1910.305(g)(2)(ii): Flexible cords were not used in continuous lengths without splice or tap:    On or about November 15, 2012 and prior thereto, in the chemistry lab a flexible cord supplying power to a portable lamp in hood #3 was spliced.  The area between the splice and the lamp was not covered with an outer sheath of insulation.   This condition exposed employees adjusting the lamp to the hazard of electrical shock.

29 CFR 1910.1450(e)(1): Where hazardous chemicals were used in the workplace, the employer did not develop and carry out the provisions of a written Chemical Hygiene Plan which was capable of protecting employees from health hazards associated with hazardous chemicals:    On or about November 15, 2012 and prior thereto, the employer had not developed and implemented a chemical hygiene plan for employees working with chemicals such as but not limited to benzene, cadmium oxide, , hydrochloric acid, sulfuric acid, triisobutylphosphine, , chloroform, nitrogen gas, carbon dioxide, acetone, and toluene.,
