29 CFR 1926.403(b)(2):    Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:     a) Power Contracting Company / 220 Scotland Road, Edinboro, PA: On or about August 22, 2012 above the data center on the first floor, North section of existing building: A temporary knockout style receptacle outlet box was on the ground. The box was used in a portable application and not utilized in accordance with its listing in labeling in that it was not designed for a portable application and was to be surface mounted.     b) Power Contracting Company / 220 Scotland Road, Edinboro, PA: On or about August 22, 2012 above the data center on the first floor, North section of the existing building: There was Type NM cable laying on the ground and routed through a hole in the floor to the temporary knockout style receptacle outlet box. Type NM cable was exposed to physical damage in that it was not protected from damage where it came through the floor hole. Type NM cable was also exposed to physical damage from construction materials, foot traffic and equipment.
