29 CFR 1926.501(b)(11):  Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:        a) Kajax Home Improvement located at the worksite on 2515 Heatherwood Drive in Findlay, Ohio:  On or about August 24, 2012, the employer did not ensure employees were protected from a fall height hazard approximately ten and a half feet above ground while conducting roofing operations on a commercial structure.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:    a) Kajax Home Improvement located at the worksite on 2515 Heatherwood Drive in Findlay, Ohio:  On or about August 24, 2012, the employer did not ensure that employees were trained in the proper application of fall protection equipment while conducting roofing operations and exposed employees to a fall height approximately ten and a half feet from the ground.
