29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    (a) On or about 08/23/2012 - 120 Park Square Lane, Madision, AL, debris such as but not limited to bricks, strapping, boards with nails, cans, bottles and other types of trash were present in the work areas.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    (a) On or about 08/23/2012 - 120 Park Square Lane, Madison, AL, the fabricated frame scaffold system employees were working from was not fully planked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcture, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:    (a)  On or about 08/23/2012 - 120 Park Square Lane, Madision, AL, the fabricated frame scaffolding system employees were working from did not have a safe means of access.  Employees were using the cross braces to access the working levels.
