Observed on or about September 05, 2012, employees were using an industrial storage rack to that was not anchored to the floor.  The industrial storage rack was used to store cleaning solution, tools, and automotive parts.  The industrial storage rack was located in an area of forklift and vehicular traffic.  Employees engaged in vehicle maintenance and tank welding activities in the main garage area were exposed to crushed-by and struck-by hazards.        b) Upper Warehouse Building, 231 York Lane, Export, Pennsylvania, 15632. Observed on or about September 05, 2012, industrial storage racks used to store pallets of petroleum products were not anchored to the floor.  The industrial storage racks were located in an area of forklift traffic.  Employees working in the Upper Warehouse Building were exposed to struck-by and crushed-by hazards.
