On or about 8/28/2012, at 1500 NW 94th Ave. in the city of Miami, FL; employer did not kept in a clean condition the following locations from water stains: Location 1: South wall under window ledge, 2nd floor at customer service cubicles. Location 2: South wall ceiling panels, 1st floor at training room. NOTE: CERTIFICATION OF ABATEMENT MUST BE SUBMITTED FOR THIS ITEM.
