29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or were likely to be present, which would necessitate the use of personal protective equipment (PPE).     (a) Chase Lumber Company, Inc., 17600 Smith Road, Aurora, CO 80011; On and before August 30, 2012, the exposing employer failed to perform a hazard assement of the chain saw operation which takes place in the main yard to determine if any additional personal protective equipment, such as chaps, were required for the chain saw operations.  Condition exposed employees to laceration and cut hazards while using the chain saw due to improper equipment.
