29 CFR 1910.151(c):     Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    (a) Furnace and Bath Area - The eyewash in the area of the phosphate and hydrochloric acid tanks had dirty water when tested making it unsuitable for flushing of the eyes, observed on or about 8-30-12.    Abatement ceritfication required.

29 CFR 1910.303(f):     Each service, feeder and branch circuit, at its disconnecting means or over current device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:    (a) New Side Sump Area -  A Square D circuit breaker box with 13 circuit breakers in the box was not marked as to the purpose of the circuit breakers.  This condition could cause additional electrical shock injury in case of a delay in turning off power in case of an accident.  This condition was observed on or about 8-30-12.     Abatement certification required.

29 CFR 1910.303(b)(2):     Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:     a) Die Shop - An improperly sized and configured strain relief (flat cable type) device was used to secure a round cable in a switch box fitting.  No conductors were exposed but this condition caused crushing of one side of the wire.  This electrical wiring method was improper and not in accordance with listing and labeling instructions.        Abatement certification required.
